What This Privacy Policy Covers

This policy covers how USTM treats personal information that USTM collects and receives, including information related to your purchase and use of USTM services. Personal information about you that is personally identifiable like name, address, email address, phone number, policy number, doctor’s name and diagnosis and that is not otherwise publicly available.

• By using any USTM on-line, in any way accessing USTM, or using other services of USTM, you are accepting the practices described in this policy and our terms of use.

• USTM may collect personal information when you (a) visit USTM on line (b) registering as a USTM patient, (d) use USTM or (e) purchase future USTM services.
• Once you register with USTM you are not anonymous to us, but your information is anonymous to any dispensary or Doctor who are members of USTM.
• USTM collects information about your transactions with us, including information about your use of services and purchase of products.
• USTM may automatically receive and record information on our server logs from your browser, including your IP address and USTM cookie information.
• USTM may use information we receive from you for any business purpose, such as to (a) fulfill your requests for products and services, (b) improve our services and customize the advertising and content you see, (c) conduct research, troubleshoot problems or resolve disputes.
• You consent to the collection and use of your information, and use as set forth in this privacy policy.

Information Sharing and Disclosure

USTM is prohibited by law and may not sell, rent, or share personal or medical information about you.

• We respond to subpoenas, court orders, or legal process, or to establish or exercise our legal rights or defend against legal claims;
• We reserve the right to send you certain communications relating to the USTM service, such as service announcements and administrative messages that are considered part of your USTM enrollment, without offering you the opportunity to opt-out of receiving them.

• Sensitive medical information and personal information, is encrypted and protected in HIPPA compliant servers for your privacy and security.
• Personal information collected by USTM is stored in secure databases, accessible only by USTM.
• We have physical, electronic, and procedural safeguards that comply with applicable regulations to protect personal information about you and your medical information.
• Patient-Doctor information and claims information are stored on HIPPA Compliant, encrypted and secured databases that follow Federal Privacy Act protocols.

Changes to this Privacy Policy

• USTM may modify, cancel or update this policy at any time in its sole discretion. Any changes will become effective immediately, and will apply to all personal information then in USTM´s possession, custody or control, including all previously-collected information.
• USTM may notify you about significant changes in the way we treat personal information by sending a notice to the primary email address specified in your USTM account, by placing a notice on our site.
